Levofloxacine po 500mg 2dd

Remarks:

Bij bewezen gevoeligheid voor zowel rifampicine als fluorquinolonen. Eventueel kan levofloxacine worden vervangen door ciprofloxacine 2dd750mg. Zie opmerkingen voor indicatie rifampicine.

General comments

  • Na initiële IV-therapie, idealiter gedurende 1 week, overleg met arts-microbioloog en/of internist-infectioloog voor switch naar oraal regime. Totale behandelduur 12 weken. 
  • Rifampicine direct starten bij bewezen gevoeligheid, dus niet wachten tot de wond gesloten is. Alleen geven bij in opzet curatieve behandeling met behoud van prothese/kunstmateriaal.
  • Als prothese/kunstmateriaal geheel is verwijderd behandel dan 6 weken als osteomyelitis (zonder toevoeging van rifampicine).

Algemene opmerkingen:

  • Vancomycine: bij voorkeur continu doseren, voorafgegaan door oplaaddosering.
  • Beta-lactam: indien mogelijk continu doseren, voorafgegaan door oplaaddosering.
  • Beoordeel gerapporteerde antibiotica-allergie aan de hand van de anamnese.

Zoektermen: PJI, kunstgewricht, FRI, fracture-related infection, fractuur, breuk, staphylococcus aureus, coagulase negatieve, CNS.
